About Our Supported Living Service

Our Purpose

Realcare Health Services Ltd provides supported accommodation and comprehensive services for young people aged 16 and above who are leaving care. We bridge the gap between local authority care and adult independence with holistic, tailored support.

Rooted in empathy, respect, and empowerment, our ethos is to create a safe and nurturing space where young people grow in confidence, learn independent living skills, and work toward meaningful futures.

What We Provide

 

Our Supported Living service provides safe, high-quality accommodation for young people aged 16 and over, creating a nurturing, supportive environment for those transitioning to independent adult living. We offer both sole and multiple occupancy accommodation options, including family-style homes where young people can live alongside peers who are also ready to live independently with support.

We undertake a complete assessment for each young person to determine their abilities and create personalised plans that focus on:

We work in strict adherence to the Care Leavers’ Charter, ensuring that all young people in our care receive a positive, consistent, and excellent standard of support. Our main aim is to provide an environment where young people can develop and enhance their independent skills while feeling safe, supported, and valued.

High-Quality Accommodation

  • Accommodation maintained, decorated and furnished to a high standard, overseen by our in-house maintenance team
  • Available 365 days a year with 24-hour support
  • Both sole occupancy and shared living options are available

Tailored Support Programmes

    • Bespoke care packages and support hours are dependent on individual needs
    • A tailored independent living programme for each young person

  • Coordination with schools, colleges, Job Centres, and training providers

Health & Wellbeing Services

  • Support to register with local health providers
  • Access to counselling and support in areas such as:
    • Drug and alcohol issues
    • Sexuality and sexual health
    • Anger management
    • Self-harm prevention
    • Safeguarding

Life Skills & Independence

  • Tailored independent living programmes
  • Practical skills development including budgeting, cooking, and tenancy management

Additional Support Services

  • Financial advice and assistance
  • Support with future housing
  • Access to advocacy services
  • Support in joining the Care Leavers Association for peer support and national guidance
